- Routine care, like an annual physical exam
- Medical treatment for nonemergency or nonurgent care
- Follow-up care if you have ongoing medical problems
- Preventive health care, like immunizations
- Patient education and counseling
- Diagnostic laboratory and X-ray test
The Patient Centered Medical Home
All Wood PCMH beneficiaries are assigned a Primary Care Manager within our Carroll Medical Home Team. Your PCM is part of a team made up of a nurse, medic, and other ancillary staff. You are at the center of that team. Our goal is to provide you with all the care that you might need, at a single site. We offer most medical services a Soldier or dependent might need. We will refer for specialty and emergency care. All regular services are available during business hours.
