The Soldier Centered Medical Home (SCMH)
All Carroll SCMH beneficiaries are assigned a Primary Care Manager (PCM) within our Carroll Medical Home Team. Your PCM is part of a team made up of a nurse, medic and other ancillary staff. You are at the center of that team. Our Goal is to provide you with all the care that you might need in a single site. We offer most medical services that a Soldier might need. We will refer for specialty care. All regular services are available during business hours.
