The Soldier Centered Medical Home
All Carroll SCMH beneficiaries are assigned a Primary Care Manager within our Carroll Medical Home Team. Your PCM is part of a team made up of a nurse, medic, and other ancillary staff. You are at the center of that team. Our goal is to provide you with all the care that you might need, at a single site. We offer most medical services a Soldier might need. We will refer for specialty care. All regular services are available during business hours.

Your Health
The SCMH strives to improve your overall health outside of clinic by providing routine preventive screenings for various age populations. Below is a list of preventative medical screenings we provide:
- Please inform your provider if you are currently prescribed any antidepressants, sleep medications or antipsychotics so we can ensure you do not lapse in medication management.
- In the interest of your health, our healthcare team will be reaching out to you when you are due for any of the above preventative screenings.

You have the right to change your PCM at anytime. Please visit the TRICARE office to begin the process.
Second Opinion
You have the right to subspecialty care and a second opinion at any time. Please discuss with your nurse or PCM.

Medical Readiness Self-Service
IMMUNIZATIONS
Walk-In Monday-Wednesday & Friday, 8:30 - 11:30 a.m. and 1 - 4 p.m.
Thursday 1 - 4 p.m.
*Soldier MUST be within no more than 72 hours prior to the listed due date for each immunization needed* No groups over five service members per unit at a time will be allowed to sign in. Maximum Immunization capacity is 40 personnel per day. Additional accommodations may be discussed with Carroll SCMH NCOIC or OIC as necessary.

Wednesday and Friday, 9 - 11:50 a.m. and 2 - 3 p.m.; Thursday, 1 - 3 p.m. (appointments also available at Camp Walker Clinic Mondays and Tuesdays)
*Testing conducted every 60 minutes for up to four Soldiers per test, at the top of the hour.
