Appropriate medical or educational personnel must complete DD Form 2792 and/or DD Form 2792-1 to verify that the condition has improved (or no longer exists). Take the completed forms to the EFMP office in your local Army Military Treatment Facility (MTF). The EFMP office reviews the information and forwards the forms to an Army Regional Medical Command (RMC). If the Family member no longer meets the medical or educational criteria, the RMC unregisters the Family member and disenrolls the Soldier. This process may take several weeks.

If a Soldier does NOT have physical custody of his/her child(ren), (i.e. is NOT the custodial parent) unregistering is appropriate. If the status is joint physical custody, unregistering is not authorized. Conversely, if the sponsor does not have physical custody, the Army EFMP is not authorized to register the Family member or enroll the sponsor in the EFMP.
Because the Family members continue to be listed in DEERS as dependents, a divorce decree, which stipulates custody arrangements, is required for unregistering Family members.
A Family member may no longer be eligible for care if they have reached the age of 21, or no longer meet the college or incapacity guidelines in DODI 1000.13, Identification (ID) Cards for Members of the Uniformed Services, Their Dependents, and Other Eligible Individuals.
Family member must submit a printout or approved DD Form 1172, Application for Uniformed Services Identification Card DEERS Enrollment, obtained from the local ID Card Section to verify that the Family member is no longer DEERS eligible
